T-SHIRT ORDER FORM

For all custom long sleeve shirts, sweatshirts, and hoodies, this form can also be used.

A non-refundable deposit of $15 is required to receive a design quote for your order. You will pay this deposit fee after submitting the form. ​This fee is applied to your total balance due.

For custom long sleeve shirts, sweatshirts, or hoodies, these forms can also be used. Your design quote will reflect your accurate apparel needs and total price.​

Turnaround times may vary depending on the workload at the time of ordering, the requested design, or even delays in communicating opinion of quote.​

If possible, complete form 2-3 weeks in advance for Single Orders and 3-4 weeks in advance for Bulk Orders.
